We are looking for an enthusiastic, passionate, self-motivated, dynamic and innovative pharmacist to provide excellent patient care in retail, care home and nursing home facilities.
Key responsibilities:
Checking prescription for accuracy, Counsel patients on prescription and OTC medications
Counsel and educate patients on health promotions and prevention activities such as immunization, provide training for medical equipment
Responsible for building and maintaining strong relationship with patient, customers and provide excellent patient focused services
Responsible for building and maintaining strong relationship with health care providers such as physicians and nurses in long term care facilities, Monitor and review patients medication profiles at regular intervals and upon request from prescribers
Interpret patients laboratory results and make suggestion for medication management accordingly, Make recommendation regarding medication treatment to health care providers based on patients medication profile
Work with other health care providers to develop strategies to meet long term care facilities goals such as maintaining medication inventory for contingency treatment
Perform drug utilization evaluations for specific drug categories and report to health care provider when appropriate
Perform and supervise compounding of sterile and non-sterile medication preparations
Must be able to perform on-call duties in the evenings and weekends
Deliver health promotion program as required
Perform educational presentations in long term care facilities for patients, health care providers such as physician and nurses
Perform educational presentation in community on various health related topics
Must be or eligible to register with the Saskatchewan College of Pharmacists
7 months to less than 1 year pharmacy experience required
Willing to express long term commitment
Willing to get Immunization and Injection certified
