The Housing First Case Management Worker will work 35 hours per week at the Saskatoon Indian and Metis Friendship Centre to providing the following Housing First services:
Assess housing barriers of individuals experiencing homelessness and develop a housing procurement, financial, and self-sufficiency case management plan with clients
Manage clients cases, follow up on advocacy and referrals for a period of up to 18 months
Initiate, collaborate and/or host front-line and housing workers partnership meetings to share best practices and identify services
Build relationships and liaise with service agencies and stakeholders such as landlords, social services, low-income housing and housing associations to support sustainable housing
Create and maintain consistent communication channels, both verbal and written, between all parties (i.e. tenants, landlords, referral sources, collaborating agencies, debtors and creditors)
Provide basic one-on-one life skill workshops based on participant needs
Provide support services and counseling to equip and empower clients to sustain housing
Interact with clients who have complex service needs (ie: related to a mental illness and /or substance abuse disorder)
Advocate for higher needs cases (social assistance cut-offs, utility cut-offs, eviction process, rentalsman hearings, food bank referrals)
Apply knowledge of residential lease contracts to educate clients of their rights and responsibilities
Minimum Job Requirements:
Knowledge in Housing First philosophy and strategies
Knowledge of tenants rights and responsibilities
Knowledge of community resources, social service agencies and landlords in Saskatoon and area
Ability to speak an Aboriginal language is an asset
Valid drivers license and a vehicle
Aboriginal candidate preferred (please self-identify)
