The Community Health Representative is responsible for assisting the Community Health Nurse in organizing, managing and delivering community health services to community residents.
DUTIES AND RESPONSIBILITIES:
- Provide information, instruction and guidance to community residents educating them in health care practices and utilization of the health care system
- Assist the community health nurse with contact tracing and other aspects of the communicable disease program including immunizations
- Provide information on Communicable Disease program and distribution of condoms
- Conduct home visits to describe, demonstrate and encourage safe health and hygiene practices
- Encourage and promote healthy lifestyle choices to prenatals, postnatals and community residents
- Assist in prenatal, maternal, well-baby, child care and other health clinics and home visits
- Assist in linking pregnant women to supports and services available to them in the community
- Provide support that enables prenatal and postnatal moms meet their identified goals (eg. transportation to clinics, prenatal classes, Food Experience and Store Tour sessions, Hospital tours, etc.)
- Distribute vouchers for milk, fruit and vegetables for prenatal and postnatal moms and provide nutrition education
- Provide basic first aid (e.g. bandages, etc.) and information to students and community residents
- Prepare and distribute printed materials/resources
- Assist the community health nurse with presentations on health and hygiene to students in the school and to adult members during community workshops
- Advise members about other available health services (eg. Regional Health Authority services, treatment centers, Saskatchewan Hearing Aid Plan, etc.)
- Serve in a liaison and interpreter role when required by community members
- Prepare an annual work plan jointly with the CHN
- Maintain strict confidentiality regarding all client information
