Description

The Quality of Life Coordinator of the Day Program is responsible for the planning and follow through of day to day programming. This includes developing a participant schedule of purposeful facility and community based activities.
Duties:
-    Provides immediate supervision and direction to support workers and participants
-     Coordinates scheduling of activities for participants and support workers
-     Produces a weekly and monthly schedule of activities for the Day Program area
-     In collaboration with the client services coordinator, will construct a program plan for each participant, including Quality of Life assessment and identification of individual goals.
-     Facilitation of group activities
-     Ensures Day Program equipment is maintained (i.e. computers, appliances, furniture etc.) and that supplies and materials are purchased and in stock (i.e. personal care room, craft supplies, food items, etc.)
-     Assists in the provision of personal care
-     Liaisons with participant support team regularly
-     Participates in program and participant meetings
-    Other duties as assigned
Qualifications:
-    Bachelor Degree from related Human Service disciplines and two years related experience. Supervisory experience also an asset.
-    Class 4 Drivers License; CPR and First Aid, Transfer, Lift and repositioning training required.
-    Knowledge of behavioral support plans and how to implement and follow through
-    Innovation along with the ability to plan purposeful, meaningful activities for individuals with varying abilities
-    Knowledge of community opportunities and service providers
-    Excellent verbal and written communication skills; proficient computer skills
-    Well-developed time management, problem solving and decision making skills
