Description

Duties and responsibilities:
The Transitions Coordinator is responsible for facilitating transition services to students with barriers who are still in school and will be entering the community and eventual employment. Develop and build linkages with service providers to ensure that youth with barriers have seamless access to suitable programs and services once finished school. Duties include:
-Provide direct support to youth participating within the transition service and their families and education professionals.
-Assist youth to develop realistic and attainable transition plans as well as assist in the process itself.
-Facilitate a curriculum for a Work Readiness course intended to develop interpersonal and vocational/employability skills.
-Develop and support community activities such as work experiences, volunteer placements and after school.
-Evaluate the effectiveness of the transition project to determine future work in this area.
-Prepare and submit a final report.

Qualifications:
-Post-secondary education in the human services field and/or three years related work experience
-Two years of experience working with youth and knowledge of barriers youth may face
-Knowledge of supported employment services and community resources
-Proven experience with program evaluation
-Use of a personal vehicle and valid drivers license
The final candidates are required to provide satisfactory police record checks
