The Community Health Representative (CHR) is responsible for directing and overseeing the daily operations, developments and administration of health programs that are community based and/or mandatory programs as outlined in the community health plan and contribution agreement delivered in the community. These programs include: Child Health Clinics (CHC), Children's Oral Health Clinics (COHC), Aboriginal Diabetics Initiative (ADI), including assistance with Discovery of Diabetics Clinics & Education Programs (DODC), Canada Pre-Natal Nutrition Program (CPNP), Family Wellness Initiatives and any additional community health based initiatives as directed.
