Scattered Sites, a project of NSN (North Sask. Special Needs) has an opportunity for the role of Outreach Worker. This position is responsible to work within a team setting providing support and measurable outcomes to the vulnerable sector that we serve.
Duties include but are not limited to:
>Providing one-on-one support in the areas of goal-setting; problem solving; communication, social and relationship skills and addiction recovery issues with the aim of supporting individuals living with addictions in their recovery.
>Organizing and conducting groups in life skills.
>Facilitate weekly program delivery through agency partnerships in the following areas:
Addictions;
Mental Health;
Methadone Maintenance;
Sexually Transmitted Infections
>Provide independent living support.
>Provide living skills advice in areas such as medication management, hygiene, and daily activity planning.
>Facilitate client's involvement in educational and vocational resources as required.
>Provide crisis intervention as required specifically as related to addictions recovery and housing stability.
>Provide transportation as required according to policy.
>Advocating in areas, or on topics such as housing that create barriers to successful outcomes;
>Assure appropriate records are completed and maintained, such as client contact notes, program reports, incident reports, and maintain statistical data on number of persons served, services provided, activities, housing stability, and results.
Qualifications:
An understanding of the population we serve along with an appropriate degree or certificate in social work, addictions or mental health, are an asset and will be given preference. The successful applicant will also have demonstrable writing and reporting skills, and will be proficient in MS Word and Excel.
A Criminal Record and Child Abuse Registry check is required with the resume. Successful candidate must hold a valid driver?s license.
