POSITIONS DUTIES (but not limited to):

- Help improve access to treatment and care and comprehensively addressing other health issues.
- Providing social support such as referrals, legal issues, childcare, housing, financial assistance and other social barriers.
- Work with young adults through seniors who have been diagnosed with a mental illness.
- Provide support to community health nurses and other care providers. ( i.e. ehealth/Telehealth)
- Plan and deliver capacity building workshops or similar activities; promote cultural competent understanding among support staff.
- Train support workers on effective communications methods when dealing with people living with HIV.
- Meeting with the clients on a scheduled basis and teaching life skills; coordinating client's confidential appointments and providing support.
- Working with the health benefit coordinator to assist with non-insured health benefits.

QUALIFICATIONS/ SKILLS/ABILITIES:

- Bachelor's Degree in Health Sciences/ Social Sciences/ Social Work/ Public Health.
- Relevant experience in First Nations community health and very familiar with First Nation concepts of holistic health and wellness.
- Knowledge of current trends, issues and promising practices regarding HIV/AIDS, sexual health and blood borne pathogens in the First Nations community (locally, regionally and nationally).
- Familiarity with co-infections (TB, Hep C and sexually transmitted infections).
- Ability to engage community and leadership to increase supports for HIV positive persons and their families.
- Demonstrated ability to work collaboratively in a multi-disciplinary team.
