JOB SUMMARY:
The Assessor/Coordinator is responsible for providing assessment, care
planning, client advocacy, and coordination or services and monitoring of care needs. As a team member, the Assessor/Coordinator Degree Senior will participate in policy and program development, health education, and promotion and liaison with other health professionals/service sectors in the community.

Qualifications:
-Baccalaureate degree in Health Sciences or Human Services
-Current Registration with a professional association/licensing body in the province of Saskatchewan
-Two (2) years clinical experience and background in Home Care
