Hours of Work:In a 6 week rotation: 28 shifts of 8.00 hours
Salary or Pay Band: $38.080 to $47.490 (6 step range)
Job Summary:
The PHC RN is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. The PHC RN promotes quality health care outcomes working within established standards for case management practice. The primary care nurse in this position will support the education, use and reporting of the CDM-QIP templates in Med Access. This position requires experience in the use of an electronic medical record. This is a regional position and travel may be required. Field hours with the possibility of evening and weekend hours.
Required Qualifications:
Bachelor Degree in Nursing
Basic Life Support (BLS)- Level C Health Care Provider (HCP) - Heart & Stroke
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Valid Class 5 drivers license
Knowledge, Skills & Abilities:
Ability to work independently and as a member of a multi-disciplinary team
Ability to work self-directed
Case management skills
Computer skills in such programs as Microsoft Office Suite (Word, Excel, PowerPoint, etc)
Excellent interpersonal and communication skills
Additional Text:
Addional requirements include:
Advanced training in chronic disease management
Knowledge and skills in "Med Access". A comparable electronic medical record may be considered.
Interdisciplinary case management experience and knowledge.
