Duties and responsibilities:
Reporting to the Quality of Life Coordinator and working as part of a team, the Quality of Life Mentor provides on-going support to participants of the program through the development and implementation of individualized strategies that address various issues. Duties include:
- Providing individual or group assistance to participants in accordance with their personal program plan and/or comprehensive behavior support plan
- Developing and implementing individualized strategies for clients of the Quality of Life program
- Providing supervision and support to the participants
- Providing transportation to community outings using an Abilities Council vehicle
- Monitoring the progress of participants and maintaining client records
- Assisting with developing and scheduling activities for the program
- Facilitating strategies with other staff members to ensure appropriate communication and support for the participant
- Assisting individuals with self-management skills and encouraging independent action
- Administering personal care to participants as required
Qualifications:
Education:
- Disability Support Worker/Rehabilitation Worker Certificate or equivalent
Experience:
- Two to three years direct experience working in the Human Services Field experience working with individuals with disabilities would be preferred
Skills:
- Excellent interpersonal skills
- Ability to express and formulate written and verbal ideas and information in a clear, concise manner
- Ability to work in a variety of work environments
- Drivers license and access to a personal vehicle
- First Aid and CPR certification
