Reporting directly to the Health Program Manager with clinical supervision provided by Saskatoon Tribal Council Focus Prevention Coordinator, the Prevention Home Visitor is responsible for carrying out the duties as outlined herein.
Duties:
1.Adhere to the Personnel Policy as set out by the Chief and Council;
2.Maintain strict confidentiality at all times;
3.Must understand, support and participate in the community enhancement initiatives in working towards the vision of Yellow Quill First Nation which improves the quality of life;
4.Provide information, counseling and support services to families;
5.Screen for the safety of the child/children such as risk of abuse and neglect and communicate risk to the appropriate party;
6.Assist in identifying the need for family support services and refer to the Prevention Worker for case planning;
7.Provide intervention as appropriate;
8.Refer reports of child abuse and neglect for investigation;
9.Carry out case plans as directed by the Prevention Worker and the clinical supervisor;
10.Maintain confidential, current, accurate, and complete case documentation to appropriate standards;
11.Conduct all tasks with regard to any and all relevant legislation, policies and procedures;
QUALIFICATIONS:
1.BISW or BSW/CSW and/or training and experience in the Human Services field is considered an asset.
2.Knowledge of case management, crisis intervention procedures, child protection, child-in-care services and protocols.
3.Must have a valid Saskatchewan drivers license, reliable vehicle and meet the required liability insurance requirements.
4.Must have a clear current CPIC (Canadian Criminal Records Check), which identifies a vulnerable sectors check.
