As a Registered Nurse, specializing in case management, you use your clinical expertise to develop comprehensive case management assessments using the case management process. You will provide the Forces member with client centered case management services to assist client's return to active duty (Return to Work program) or to civilian life. You will be responsible for coordinating and monitoring the care of the Forces member who may have complex health care needs. You will arrange health service provider's appointments and other services for the Forces member. You will act as a focal point of contact for the Forces member and engaging them in the case management program up to the return to work or final release date.
