DESCRIPTION:  
The Support Worker is directly responsible to the
Senior Support Worker or Group Home Manager. The
Direct Support Worker's role is to provide
companionship, support and attendant care to
individuals in social or community settings. The
Direct Support Worker is also part of the dynamic
team that ensures the smooth day to day operations
of Clare Parker Homes Inc.

QUALIFICATIONS:
- Education and/or related experience in Human
Services.
- Current CPR and First Aid
- Valid drivers license
- Good communication and interpersonal skills
- Ability to work effectively as a team member
- Current police record check, with a vulnerable
sector search

RESPONSIBILITIES:
Participant Support  
- Build a relationship based on friendship
equality and respect
- Administer medication if requested by the Senior
Support Worker or Group Home Manager
- Follow an appropriate schedule of activities as
established in conjunction with the Senior Support
Worker or Group Home Manager
- Facilitate family involvement where appropriate
- Household chores as needed
Record Keeping
- Ensure the confidentiality of participants
records and history
- Preparation of reports as directed by the Senior
Support Worker, Group Home Manager, or Management
- Maintain appropriate client records and prepare
client reports as directed
- Attend staff and support team meetings as
directed by the Program Coordinator or Executive
Director
- Maintain a positive relationship with the
support team
- Additional responsibilities as requested by the
Senior Support Worker, Group Home Manager, the
Program Coordinator and/or Executive Director

HOURS OF WORK:
Casual, Evenings & Weekends, Hours Vary.
