Description

The HCC Nurse (RN) works under the general supervision of the Health Program Manager. The HCC Nurse (RN) uses her/his nursing/teaching skills to effectively assist HCC clients to attain their optimum level of independent functioning in activities of daily living. The HCC Nurse (RN) also manages and coordinates the Aboriginal Diabetes Program (ADI).
Duties Related to Program Service Delivery:
The HCC Nurse is responsible to provide active and effective nursing service following the clients care plan.
This is done by:
Adhere to the Personnel Policy Manual as set out by the Yellow Quill First Nation Chief and Council;
Providing case management services;
Continually assessing the clients health status;
Consulting with the clients physician or other health agencies/professionals;
Providing supervision to auxiliary staff;
Maintaining current skills.
Duties Related to Program Development
As a member of the HCC staff, the Nurse will:
Keep immediate Supervisor/delegate informed of any concerns;
Participate, as requested, in any Band/Tribal Council initiatives/activities;
Seek additional funding through grants and fund raising activities to enhance the Home Care program;
Develop the ADI work-plan and carry out the activities once approved.
Qualifications:
Graduate of an accredited School of Nursing.
Must be a Registered Nurse/Registered Psychiatric Nurse, licensed to practice with their Saskatchewan Professional Association.
Maintain a valid Saskatchewan drivers license, have access to a reliable vehicle and meet the community insurance requirements.
The ability to speak Saulteaux, an effective understanding of Saulteaux culture and spirituality, and experience working with First Nations communities is preferred.
Must have a clear current CPIC (Canadian Criminal Records Check), which identifies a vulnerable sectors check.
