Primary Health Care (PHC) is a delivery model that utilizes integrated, interdisciplinary teams of health professionals and community members to provide high quality service as close to home as possible, at the most appropriate time, by the most appropriate person. Under the direction of the Director of Primary Health Care, the Primary Health Care Team Manager is accountable for the PHC team members in the Meadow Lake, Goodsoil and Loon Lake area. This position provides leadership in team building, clinical excellence and interdisciplinary team practice.
The successful candidate must possess the following qualifications:
Bachelor of Science in Nursing and eligible for licensure with the Saskatchewan Registered Nurses Association.
Comprehensive understanding of Primary Health Care and the role of the Registered Nurse on the PHC team.
Demonstrated management, leadership and/or supervisory experience is required.
Strong facilitation skills with an emphasis on team development; an effective change agent who is a team player with excellent written, verbal and interpersonal skills.
Proficient with computer software (Word, PowerPoint, Excel, Outlook).
A valid drivers license is required.
