Description

In consultation with the Community Health Program Manager will perform the following tasks:
Knowledge of case management and crisis intervention
Collaborate with Health Center team, and other professionals when appropriate through consultation and case conferences to evaluate clients' medical or physical condition and to assess client needs.
Refer clients to appropriate community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, adequate housing, job placement or education.
Plan and conduct workshops for clients and family members as needed to combat social problems, prevent substance abuse that deemed appropriate by plan-of-care developed by clients.
Counsel clients in individual and/or group sessions to help them overcome dependencies, recover from illness, and adjust to life.
Hospital visits, home visits, off-site external visits when possible
Monitor, evaluate, and record client progress
Participation in the development, implementation and maintenance of appropriate procedures in accordance with any appropriate legislation and STC Health Center policies.
Participation in the evaluation of the program to ensure maintenance of standard of care.
Complete individual monthly statistics in accordance with STC procedures and standards
Complete monthly reports with statistics, clients issues, trends and recommendation.
Moderate risk of exposure to disease or illness through provision of client care
Services provided in clients home environment and/or community setting which presents many unknown variables
Exposure to clients who may have impaired judgment due to emotional distress, mental illness or intoxication.
Must be willing to work flexible hours as assigned by management, which may include some evening and weekends
An acceptable Criminal Record Check and Child Abuse Registry Check
Valid drivers license and access to a vehicle
