DUTIES AND RESPONSIBILITIES
Develop and/or maintains professional competencies required for nursing practice specific to immunizations, HIV/AIDS, STIs and those at risk for infection;
Provide input to the multi-disciplinary team to ensure development and continuity of client care;
Coordinate implementation of plans to connect clients to community treatment and supports;
Conduct nursing assessments and history taking;
Provide direct care to clients within the community as appropriate;
Accurately monitor, record and report client status and progress;
Maintain records and documentation;
Educate and counsel clients and families regarding disease processes and healthy lifestyles;
Offer HIV testing services and referral as appropriate;
Manage notification and referral process as outlined in The Public Health Act;
Act as a advocate on behalf of clients, their families and the vulnerable population;
Facilitate condom distribution and needle exchange;
Work collaboratively with team members on overall program development and evaluation; and
Network with employees of other health care agencies.
SKILLS AND QUALIFICATIONS
Registered Nurse Qualification, BScN preferred;
Must be registered or eligible for registration with SRNA;
Level C, Basic Cardiac Life Support;
Strong assessment skills;
Demonstrated success in connecting with vulnerable populations;
Understanding of harm reduction strategies;
Ability to work independently and as member of a multi-disciplinary team;
A working knowledge of a First Nations language and an understanding of First Nations culture and traditions would be an asset;
Demonstrated strong interpersonal and collaborative communication skills;
Ability to work in a flexible, challenging environment;
Basic computer skills; and
Must have a valid Saskatchewan drivers license and access to a reliable vehicle
