Job Description:
As an integral member of the Primary Health Care Team this individual will use the client ( or patient/family)-centered approach to facilitate behavioral health changes through screening, assessments, brief interventions and education. A primary function of this individual will be to hear the clients voice and assist them in achieving their health needs through the development of strong relationships and individualized holistic care plans. Working in a multidisciplinary environment including, but not limited to Physicians, Registered Nurses, Pharmacist, Administrative Staff, and other professionals and agencies within the community. The incumbent is expected to employ an effective combination of Saskatchewan Health Authority vision, mission, and values including cultural safety combined with guidance from colleagues, personal experience, post-secondary education and specialized training to assist patients in their efforts to sustain balance in all aspects of their lives.

Education
- Bachelor of Social Work (BSW) degree
- Must be a member of or be eligible for and able to maintain current membership with the Saskatchewan Association of Social Work (SASW).
Experience
- Experience working in partnership with the Aboriginal community.
- Experience in assessment techniques appropriate to vulnerable populations including those with Hepatitis and HIV
