The Housing Support Worker reports directly to the Team Lead, Housing Programs. The Housing Support Worker is responsible for the provisions of housing stabilization services for those who are homeless or at risk of being homeless. This position will provide an expansion of basic needs services and expansion of intensive case management based out of the Newo Yotina Friendship Centre that integrates with existing internal and external community resources.
Duties Include:
Conduct general in-take and ensure the Vi-SPDAT pre-assessment tool is complete.
Receive and provide referrals through the CAR (Coordinated Access) online program.
Screen, assess, risk planning and implementation through case management reporting.
Schedule and attend house viewings, provide transportation.
Assist with tenant-landlord relations.
Assist with rental payments, utility set-up and basic advocacy services.
Provide in home life skills mentoring.
Complete other duties and activities as required.
