Join our Team as a Family Therapist !

Position Overview

As a Family Therapist, you will provide essential in-home support, including crisis intervention, counseling, psychoeducation, and skill-building to help families thrive. Youll conduct assessments, create customized treatment plans, and deliver culturally responsive care centered on each familys strengths. This role includes 24/7 on-call support, ensuring families have access to care when they need it. Youll also collaborate with community agencies to implement recommendations that build resilience and healing. Our approach is inclusive, family-centered, and anti-oppressive, with training provided in our core Family Program model. Duties and Responsibilities

Services
     * Collaborate with families and stakeholders to co-create individualized treatment plans and intervention strategies.
     * Lead the implementation of treatment plans, ensuring all documentation is complete and feedback is shared with relevant parties.
     * Provide education, support, and counseling to participants, offering intensive individual, family, and couples therapy.
     * Conduct risk assessments that cover areas such as family safety, child welfare, mental health, domestic violence, and addictions.
     * Collaborate with family members to create safety plans that leverage personal and community resources.
     * Assess and respond to suspected or confirmed cases of abuse and develop safety and protective plans.
     * Lead planning conferences, consultations, and meetings with healthcare and social service professionals.
     * Facilitate holistic aftercare planning, working closely with families and community agencies to support long-term success.
     * Provide 24/7 therapeutic on-call crisis support and deliver follow-up care to promote post-crisis recovery.
