The PLWA Network is an Indigenous community-based, non-profit organization that provides care, support and education while promoting wellness to people affected by HIV/AIDS.
Under the direction of PLWA Network, employees of the transition house will collaborate with tenants to determine appropriate individual needs and situational requirements as they overcome barriers in their transition from homelessness into independent living.
Duties include but are not limited to:
-Provide support to persons experiencing mental health and addictions struggles with culturally competent and culturally safe support services.
-Implement the delivery of systems-gap services.
-Assist in case-management, data entry and reporting
-Conduct person-centred, strengths-based assessment and immediate needs assessment and provide appropriate supports and services and/or referral to community-based resources.
-Assist with treatment and recovery planning, including providing transportation to AA/NA meetings and cultural programming
-Assist in conducting individual plans as needed with each tenant.
-Liaise with other social services agencies and health care providers working with persons accessing services.
-Assist in providing transport to tenants as needed to medical appointments, methadone, etc.
-Assist in housekeeping duties in office and suites as needed.
Ideal candidates must:
-Must be able to work shift work
-have reliable vehicle and class 5 license
-Submit a criminal record and vulnerable sector check
-Have some Knowledge of concurrent disorders
-Cultural Awareness
-Comfort working with target populations
-Knowledge of Microsoft and Excel
