The Coordinated Access (CA) By-Name List Data Administrator is responsible for maintaining the By-Name List and matching clients on the By-Name List to the appropriate housing and support program vacancies in the Coordinated Access inventory, based on community-wide standards for prioritization.
Primary Responsibilities:
Work with (HIFIS) The Homeless Individuals and Families Information System Lead and community partners to validate the quality/completeness of the By-Name List and its subsets
Maintain complete, accurate, and comprehensive By-Name List and real-time CA Partner vacancy inventory
In collaboration with CA and HIFIS Leads participate in data audits and data quality and verification processes
Monitor the By-Name List to identify clients experiencing homelessness in Regina who are not engaged in the CA process, and work with community partners to engage client
Receive referrals directly and/or refer and match clients to vacancies within the Coordinated Access Regina inventory
Communicate with frontline workers/case managers in partner agencies to confirm client consent and readiness to accept a referral, including document readiness
Ensure ongoing engagement with clients on the By-Name List to maintain up-to-date contact and prioritization information
Collaborate with CA team, ensure CA inventory partners are abiding by written CA policies and procedures
Ensure confidentiality and that all operations related to CA are done in accordance with relevant privacy legislation
