The Mental Health & Addictions Community Outreach Worker is responsible for providing holistic care to BOCN community members who are impacted by problematic substance use and mental health challenges.
The Mental Health & Addictions Community Outreach Worker will provide services including counselling to adults, advocacy, case management, addiction assessments, aftercare planning and
program facilitation. The Mental Health & Addictions Outreach worker will facilitate the detox and treatment application process. This position is part of a multi-disciplinary team within the scope of the mental health and addiction program. Multi-disciplinary work will include program support to other members of the mental health team, as needed
