SUMMARY OF POSITION:
Kahkewistahaw First Nation is developing its Family Services Program. The Family Services Support Worker works directly with the Family Services Team and reports to the Prevention Manager. Who will also support families and/or individuals mentally, emotionally, spiritually and physically.
DUTIES & RESPONSIBILITIES
Support families and individuals completing healing plans.
May provide transportation when needed.
Complete welcoming sessions and healing plans.
Complete reports when needed.
Track family and individual demographics.
Communicate daily with family and/or individuals.
Connect families and/or individuals to other organizations.
Connect families and/or individuals to healing resources.
Complete home visits with families and/or individuals.
Organize and file family and/or individual healing plans.
Assist in the development, implementation, and evaluation of programs and services that address Kahkewistahaw First Nation community needs.
Follow all Kahkewistahaw First Nations policies and procedures.
Able to work evenings and weekends.
Other duties as assigned.
