The Community Outreach Response Team is a dynamic team that delivers intensive, rapid wrap around care to support persons living with HIV who are struggling with addiction and other psycho-social issues. Through a rapid response and intensive case management approach CORT will improve the health and social outcomes for this patient population.
Job duties of the successful applicant include:
Work with people living with HIV/AIDS who are also affected by mental illness, addictions, poverty, homelessness and other psychosocial factors.
Coordination of psychosocial and medical components of client care plans, facilitate care and referrals to appropriate agencies to best meet client goals.
Work closely with health care professionals in the management of people who have complex health and social care needs.
Support clients in addressing barriers to stabilization such as housing, addictions and income assistance and obtaining identification.
Coordinate and transport to medical, social and legal appointments to reduce barriers to accessing care.
Ensure all outcomes and interactions are recorded, measured and documented in client charts as per the direction of the Executive Director.
Qualifications
Degree in social work, addictions or related field
Background working in Mental Health and Addictions
Cultural sensitivity of First Nations issues
Extensive knowledge of community resources with an emphasis on life skills support, housing services and how to access and advocate for these services on behalf of clients
Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds.
Understanding of the principles of Trauma informed Care and Harm Reduction
An understanding of the issues related to HIV
Communication, conflict resolution and interpersonal skills
Valid drivers license & vehicle for client transportation
