Description

Responsibilities:

Client Support:

Provide direct support to clients with various and individual needs.
Assist clients in daily living activities, such as personal care, meal preparation, and mobility.
Provide companionship and support to clients
Administer medications as prescribed by healthcare professionals
Assist with meal preparation and feeding, ensuring dietary requirements are met
Perform light housekeeping duties to maintain a clean and safe environment
Escort clients to appointments or social outings
Document client progress and report any changes or concerns to the supervisor
Foster a supportive and empowering environment for clients to thrive.
Employees may be exposed to the risk of violent (behavioral) situations in which training reinforces safety concerns and assists in precautionary measures.
Assessment and Planning

Conduct assessments to understand clients unique requirements and develop personalized care plans.
Collaborate with other team members to ensure personalized support for each client.
Monitor progress and adjust care plans as needed.
Advocacy:

Advocate for clients rights and access to community resources.
Work closely with families, caregivers, and external agencies to enhance clients quality of life.
Attend meetings and represent clients interests.
Documentation:

Maintain accurate and detailed records of client interactions, progress, and incidents.
Complete necessary paperwork related to client care and funding requirements.
