Job Description
Reporting directly to the Sohkayawin Team Lead, the Intake + Diversion Worker is responsible for conducting CAR intakes and SPDATs, supporting Diversion clients and connecting to services for those who are homeless or at risk of being homeless. This position will provide an expansion of basic needs services and expansion of intensive case management based out of the Newo Yotina Friendship Centre that integrates with existing internal and external community resources.
Responsibilities:
Conduct CAR in-takes and ensure the SPDAT pre-assessment tool is complete.
Screen and assess at risk clients. Connect those who are homeless to shelter and warm-up spaces, as well as other support networks.
Assist Four Winds Housing Support Workers with transportation to house viewings and appointments for their clients.
Assist Diversion clients that require one-time supports to maintain or obtain stable housing.
Promote the Coordinated Access Regina system and help community members understand and connect to it. Provide communication between clients on the BNL and CAR.
Work alongside other NYFC program supports such as Resolution Health Support and cultural support to connect community members to desired support networks.
Maintain and report on program statistics and outcomes; adhere to all HIFIS database requirements.
Adhere to organizational policies and procedures, as well as all Coordinated Access Regina policies and procedures.
Work in a team environment to promote centre functions and cleanliness of the building
