The Cognitive Disability Strategy (CDS) is a provincial initiative of the Government of Saskatchewan meant to provide services to address the unmet needs of people with cognitive disabilities and their families. CDS Saskatoon delivery partner, Inclusion Saskatchewan (INSK), is seeking a temporary Cognitive Disability Consultant (CDC) to provide support and services in the Saskatoon Region up to March 31, 2025, with the possibility of extension.
About the Role
Do you have experience working with people with disabilities and their support teams? Do you have a talent for connecting people with services and navigating complex systems? The Cognitive Disability Consultant is a systems navigator connecting people with services, building community capacity, and supporting people in accessing funding through the Cognitive Disability Strategy (CDS).
Key Responsibilities
Support people with cognitive disabilities and their support teams in accessing funding through CDS when their needs are unmet by other services and supports in the community.
Using a Person-Centred approach, work with people with cognitive disabilities, their support teams, community organizations, and partners to identify potential supports, services, and funding to meet the persons needs.
Support team leads in developing integrated support teams and creating individual support plans that have measurable goals and use a Person-Centred approach.
Build community capacity by enhancing the knowledge and skills of support teams and service providers in meeting the needs of persons with cognitive disabilities.
