Community Inclusion Support Worker provide resources and practical supports to individuals with intellectual disabilities who are transitioning to an independent life in the community(supportive Independent Living Program). Upon request they may provide support in areas determined by the individual or in some of the following areas:
Financial Management
Home Management
Community Resource Awareness
Meal Planning
Peer Support
Information Networking
Promotion of Self Care
Advocacy
Leisure Activities
Encouragement of independence
As a Community Inclusion Support Worker you will
Demonstrate initiative, be resourceful and creative.
Work independently and collaboratively within the team and with the organization.
Design, implement and monitor progress of activity and recreational programs consistent with The individual's Person Centered Plan that will enhance physical, mental and psycho-social well-being of the individual.
Applicants must have a valid Saskatchewan driver license with access to a vehicle and be willing to work a flexible 40-hour week.
Support other relevant duties that may be developed or assigned by the Program Manager.
