Description

We are looking for a responsible, highly skilled, experienced and enthusiastic professional to provide enhanced service coordination to children and families, and work with families to help them at many points along the health care continuum. This may include referrals to specialist for their children, understanding treatment and care options, understanding insurance benefits, serving as coach and quarterback of their health care team, working with family members and caregivers, mobilizing resources, managing medical paperwork and finding shelter/housing options If you feel like this would be a good fit for you and your career ambition, we encourage you to apply!

About the position: The JP Navigator will  identify children with unmet needs to facilitate early intervention and timely access to services and resources; Enhance opportunities for the success of at-risk clients and families, Locate resources, Identify options and available supports (JP Group and Individual Applicants Process), Build Case Management Capacity through the development of policies, procedures, and protocols related to screening and assessment, service planning, and monitoring and evaluating of services plans and staff training; Provide a collaborative, client driven system to guide children and their families throughout a comprehensive array of services; Establish multi-disciplinary hubs for families when the services they require involves a number of professionals.
