Facility: MENTAL HEALTH OUTPATIENT
Job Description: Reporting to the Director of Therapies, the incumbent is accountable for the provision of quality occupational therapy services in the Five Hills Health Region. The location of service may be in acute care, outpatients, long term care or the community of the Five Hills Health Region. The incumbent assesses clients to determine the level of function and initiates, monitors and evaluates a program designed to improve or maintain client abilities and independence.
Required Qualifications

Bachelor Degree in Occupational Therapy
Registered or eligible for registration with the Canadian Association of Occupational Therapy
Registered or eligible for registration with the Saskatchewan College of Occupational Therapists

Successful candidates external to SHA are responsible to obtain and provide an original Criminal Record Check (CRC) & Vulnerable Sector Check (VSC) from their local police service, RCMP or previous country/countries of residence that is dated within the past six (6) months that is satisfactory to the Saskatchewan Health Authority  The Saskatchewan Health Authority (SHA) requires a criminal record check (CRC) from every potential SHA team member. The CRC must be dated within the past six (6) months and be satisfactory to the SHA. A vulnerable sector check (VSC) is required for individuals providing patient/resident/client care to vulnerable persons.
