Facility: Kindersley and District Health Centre
Job Description: The Mental Health Therapist - Staff is an integral member of the Primary Health Care Team and works in part of the network team in the medical home model.  As a member of a multidisciplinary primary health care team, the incumbent is primarily responsible for behavioral health supports for a number of conditions under the care of the Primary Health Care Provider. The services to be delivered through this position include screening functional assessment and develops behavioral change plans in conjunction with the Primary Health Care Provider.   The Mental Health Therapist - Staff develops and implements a behavioral healthcare plan and makes referrals as required.  
Required Qualifications

Bachelor Degree in a Human Services Field.
Current or eligible registration with the Saskatchewan regulatory body associated with affiliated profession
Valid Saskatchewan Driver's license
Experience
Experience in brief treatment modalities in PHC setting is an asset
Experience with a combination of clinical interventions in relation to Primary Health Care, Integrated Care or Community Health Care is an asset
