Job Description: The educator integrates skills from case management theory, best practice standards and principles of adult learning in the provision of education for CPAS client care coordinators.  The educator will provide leadership to CPAS staff that is visible and proactive and promotes the delivery of skilled case management services.  This is achieved through the application of best practice standards which includes assessing learning needs, planning, implementing and evaluating orientation, in-servicing and other educational programming; acting as a clinical resource; promoting client and family centered care; partcipating in committee work as assigned; leading continuous improvement activities; developing and maintaining internal educational resources and materials.  The Educator is responsible for ensuring that case management training is in alignment with the organization's mission goals, values and strategic plans to promote high quality case management services.

Required Qualifications

Bachelor Degree in Nursing or Bachelor Degree in Social Work or Bachelor Degree in Physiotherapy or Bachelor Degree in Occupational Therapy
Registered or eligible for registration with the appropriate licensing body
Experience
Requires a minimum of two years of recent (within the past four years) case management experience with CPAS.  Community Case management experience in CPAS is also an asset.

Advanced clinical skills, knowledge, and abilities required for competent case management practice.
