Job Description: Reporting to the Primary Care Manager and in collaboration with the primary health care clinicians and team, this position supports effective relationships between the client and the health care team. The position works within the 3 pillars of the chronic disease management model to support client self-management of their condition, support the provision of evidence based care as outlined in the care pathways for key chronic
diseases and supports access to services that will positively influence the client's health.
Works in conjunction with the client, family and other care health providers, to develop a client care plan / client self-management plan to facilitate active client involvement in the management of their long-term health conditions. Demonstrates the ability to utilize the nursing process, knowledge
and skills to educate, treat, promote health and prevent illness.
Qualifications
- Current registration with the SRNA and licensed as an RN Baccalaureate Degree in Nursing preferred
- Three years full time experience as a registered nurse working primarily with clients with chronic illnesses like Diabetes, HTN, COPD, and CAD
- Valid driver's license, vehicle and ability to travel
- Strong communication and public relations skills
- Knowledge of community needs assessment, community development, and health promotion preferred
Required Qualifications
Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing
