Job Description: THE ASSESSOR / COORDINATOR - DEGREE IS ACCOUNTABLE FOR THE ONGOING DETERMINATION FOR THE CARE NEEDS OF THE HOME CARE CLIENTS OF THE PAPHR, THE COLLABORATIVE COORDINATION OF SERVICE AND THE CASE MANAGEMENT PROCESS.  THE DELIVERY OF CARE IS CONSISTENT WITH THE POLICIES, VISION, MISSION AND HEALTH OUTCOME GOALS OF THE REGION. THIS POSITION REPORTS DIRECTLY TO THE DESIGNATED CTM (COMMUNITY TEAM MANAGER).
Required Qualifications

Degree in a health or social science related field.
Registered or eligible for registration with the appropriate licensing body
Experience
Experience in Palliative Care considered an asset.
Knowledge, Skills and Abilities

Ability to work well with different types of personalities in various settings.
Advanced Computer Skills
Critical thinking ability
Good communication skills - verbal and written.
Good organizational skills and able to work independently and in group settings.
Good public relation skills and ability to establish and maintain effective work relationships
Knowledge of Assessment and development care plans.
Knowledge of assessment processes and terminology.
Valid driver's license, reliable vehicle
Other Information

Valid class 5 Saskatchewan Driver's license and access to a reliable vehicle.
