Job Description: The Clinical Nurse Educator (CDM) integrates in-depth clinical expertise and experience, knowledge of nursing theory and the principles of teaching and learning, in the provision of chronic disease education, and quality care for clients, and families.
The Clinical Nurse Educator (CDM) is responsible for assessing learning needs, developing, implementing and evaluating best practice/educational/counseling activities for clients, and families, health in one or more of the following areas: cardiovascular, diabetes, and general respiratory.
The Clinical Nurse Educator (CDM) collaborates with the multidisciplinary team members, inpatient/outpatient/hospital departments/community agencies to establish partnerships, initiating referrals to community services as required.
The Clinical Nurse Educator (CDM) uses a consultative approach, implementing accepted standards and guidelines and current research findings to advocate for continuity and coordination of care.
Teaching can occur in inpatient, outpatient, specialty clinics, physician clinics or community settings, with individuals or groups.
The Clinical Nurse Educator (CDM) participates in research, program quality monitoring and improvement related to Chronic Disease Management using National and International clinical standards of practice in collaboration with physicians and multidisciplinary team members.
Reports to the Manager Chronic Disease Management.
The Incumbent is self-directed & requires minimum supervision.
Required Qualifications
Bachelor Degree in Nursing or Bachelor Degree in Psychiatric Nursing
Basic Life Support (BLS)
Certification in RESP Trec/Asthma Trec or required to obtain within 6 months of employment
Current licensure with the College of Re
