Under the direction of the Health Director, the Home care coordinator provides supervision and direction to the Home & Community Care Program. The main role of this position is to ensure that effective, efficient service is available to community members who require assistance to continue living independently.The HCM is also responsible for program development, quality assurance and staff supervision.
