Job Description: Reporting to the Manager of Mental Health and Addiction Services, the incumbent is responsible for the screening and coordination of initial service requests for all programs within Mental Health and Addictions Services in the former health region. The Intake Facilitator provides screening, triage, assessments for urgency/risk, and coordinates case assignments. The position facilitates crisis response and short-term support for services to clients presenting with a variety of mental health and addictions issues. The Intake Facilitator provides consultation to Community Agencies and Programs regarding services available in Mental Health and Addiction Services.
Experience
REQUIRED QUALIFICATIONS:
- One of the following: Bachelor of Social Work, Bachelor of Arts (Social Sciences), Bachelor of Science in Nursing, Bachelor or Diploma of Psychiatric Nursing or a Addiction Counseling Degree
- Current registration in corresponding professional association.
- A drivers license and ability to travel.
EXPERIENCE:
- Previous experience in the Mental Health and Addiction Services.
KNOWLEDGE, SKILLS & ABILITIES
- Ability to perform intake assessment procedures, including screening, triage, and short term treatment of clients experiencing a wide range of mental health and addictions difficulties. This involves the ability to evaluate complex situations, provide crisis response services, and/or brief treatment and to plan other appropriate interventions or referral services.
