The Muskoday Community Health Centre is seeking the employment of a Home Care Nurse.
This is a Full-Time temporary position. Our hours of operation are Monday through Friday,
from 8:00 to 12:00 and 1:00 4:30 p.m.
The duties of the Home Care Nurse will include but are not limited to:
Provide safe competent nursing care which meets existing standards;
Develop and maintain a close working relationship with the Director of Health, Clinical
Supervisor, staff and community members;
Assess, plan, design and implement Home Care Plans, programs and educational
opportunities as required;
Develop and implement the workplan for the Aboriginal Diabetes Program;
Work with the Director of Health to develop yearly budgets for the Home Care and ADI
Programs;
Supervision of Home Care Aide and Chronic Disease Management Coordinator;
Provide written and electronic Home Care Reports and stats as required;
Provide monthly written reports to the Health Committee;
Prepare an annual report for community members;
Function effectively as part of the Health Team;
Requirements:
A Registered Nurse in good standing with the SRNA;
Experience working in the Home Care field would be a definite asset;
Strong case management and client assessment skills;
Must be trustworthy, respectful, kind, and flexible;
Must be able to work as a team player and independently with a holistic approach;
Excellent interpersonal and communication skills;
Foot Care Training, CPR, First Aid, and AED would be an asset;
Knowledgeable with computers and possess great organizational skills;
Must have a drivers license and a reliable vehicle;
Must provide a current Criminal record with Vulnerable Sector check
