Summary:
The DSW is responsible for planning and providing individualized support and to ensure that the privacy and dignity of all participants is respected. Following established person-centered plans, the support they provide assists participants to maximize their potential and contributes to a positive quality of life. He/she acts as a role model and an advocate for participants as well as an ambassador for the company.
Duties:
1. Develop effective working relationships with all staff and participants.
2. Assist to develop and implement care plans and schedules for participants requiring personal care.
3. Maintain/improve skin integrity by providing support to participants with dressing, grooming and hygiene activities.
4. Offer guidance and support to encourage independence and assist participants to achieve personal goals.
5. Provide feedback for Care Plans; monitor plans for relevance and participation.
6. Documentation and reports.
7. Participate as an active team member.
8. Administer and record us of prescribed medication and tube feeds as necessary.
9. Provide laundry service.
10. Maintain a clean and safe work environment.
11. Cross-train for other positions as deemed necessary.
12. Other duties as assigned.
Preference given to applicants with Personal Care Aide, Disability Support Worker or equivalent training in a related field.
Must provide a satisfactory Criminal Record Check with Vulnerable Sector Check prior to employment.
